چکیده

Objective: The purpose of this study was to investigate local paraspinal cutaneous temperature (CT) modifications after spinal manipulative therapy at L5. Methods: Twenty subjects with acute low back symptoms were randomly assigned to either a treatment or a sham group (n = 10 per group). Subjects underwent an 8-minute acclimatizing period. Temperature was measured bilaterally with infrared cameras at the L5 level. In the treatment group, a traditional chiropractic manipulation (lumbar roll technique with a pisiform contact on the ipsilateral mamillary of L5) was delivered, whereas with the sham group, the same technique was used, but no thrust was applied. Cutaneous temperature control measurements were taken 2 minutes before (t−2) and immediately after the intervention (t0) and at 1, 3, 5, and 10 minutes postintervention (t1, t3, t5, and t10, respectively). Results: At t0, CT in the treatment group on the treatment side (ipsilateral side) warmed up by 0.2°F, whereas in the sham group, there were no significant temperature modifications on either side. At t3 relative to t0, CT in the treatment group on the treatment side warmed by approximately 0.6°F, whereas the contralateral side (nontreatment side) cooled. In the treatment group, significant differences were noted between sides (F = 13.36, P = .002, P = .932) and sides × times (F = 2.97, P = .016, P = .838). Conclusion: The effects of a lumbar spine manipulation appear noticeable by changes in paraspinal CT measurements at the level of L5. However, the meaning and mechanisms of CT modifications at L5 are still being investigated. Advances in Paraspinal Thermographic Analysis

Basic theory and common practice in temperature-based chiropractic assessment procedures and instrumentation are reviewed. Segmental spinal thermoregulatory function is presented as a refining mechanism for hypothalamic core-temperature control. Spinal nerve-cell-body function is suggested as the primary mechanism responsible for the asymmetries and anomalies commonly observed in paraspinal and...

Spinal manipulation alters electromyographic activity of paraspinal muscles: a descriptive study.

OBJECTIVE To examine the effect of spinal manipulation on electromyographic (EMG) activity in areas of localized tight muscle bundles of the low back. METHODS Surface EMG activity was collected from 16 participants in 2 chiropractic offices during the 5 to 10 minutes of the treatment protocol. Electrodes were placed over the 2 sites of greatest paraspinal muscle tension as determined by manua...

Effect of spinal manipulation on the development of history-dependent responsiveness of lumbar paraspinal muscle spindles in the cat.

We determined whether spinal manipulation could prevent and/or reverse the decrease and increase in paraspinal muscle spindle responsiveness caused respectively by lengthening and shortening histories of the lumbar muscles. Single unit spindle activity from multifidus and longissimus muscles was recorded in the L6 dorsal root in anesthetized cats. Muscle history was created and spinal manipulat...

The effect of spinal manipulation impulse duration on spine neuromechanical responses.

INTRODUCTION Spinal manipulation therapy (SMT) is characterized by specific kinetic and kinematic parameters that can be modulated. The purpose of this study is to investigate fundamental aspects of SMT dose-physiological response relation in humans by varying SMT impulse duration. METHODS Twenty healthy adults were subjected to four different SMT force-time profiles delivered by a servo-cont...

P01.23. Effects of unilateral facet fixation and facetectomy on muscle spindle responsiveness during simulated spinal manipulation

Purpose There is mounting evidence that identification of spinal joint hypoand hypermobility along with correspondingly tailored manual therapy treatment approaches for these clinical subgroups can lead to more successful therapeutic outcomes. Subjects with spinal joint hypomobility had higher therapeutic success rates with spinal manipulation than subjects with spinal joint hypermobility (Frit...
